Deformity is an AI form builder for creating conversational forms, surveys, lead capture flows, quizzes, feedback forms, and payment forms. It addresses the trade-off between rigid, structured forms and open-ended conversations by letting builders switch between AI mode for dynamic, adaptive questioning and static mode for precise, structured questions within the same builder. Forms can be embedded on websites and Notion pages or shared as standalone pages.
Key capabilities include branching logic described in natural language, file uploads, payments via Stripe, submission periods, submission limits, post-completion redirects, and built-in analytics for submissions and conversion rates. It supports 120+ languages, pre-filled answers, hidden values, e-signatures, NPS, rankings, and ratings. Integrations include direct connections to Google Sheets, Slack, Discord, Notion, Google Tag Manager, Google Analytics 4, Meta Pixel, and Stripe, plus Zapier, Make, and n8n for broader automation. An External API and an MCP server for AI assistants such as Claude, ChatGPT, Cursor, Perplexity, and Gemini are included for all users.

It offers a free plan with unlimited conversations, all question types, payments, branching logic, integrations, and API/MCP access. A Pro edition adds AI conversation allowances, custom domains, team seats, instant responses, removal of branding, and unlimited file upload sizes, with monthly and yearly billing.
